I use the following, and have done successfully for a long time:
No modifier:
left click - Healing Wave
right click - Riptide
MB3 - dispel
MB4 - Unleash Elements
SHIFT modifier:
left click - Healing Surge
right click - Greater Healing Wave
MB3 - Earth Shield
CTRL modifier:
left click - Chain Heal
Other binds:
Shift-Space - Ascendance
F - Healing Rain
Shift-T - Mana Tide
(other cooldowns set to similar Shift-LETTER combos)
How's that?

Your keybindings are your preference Pick what ever you feel is the "right button" that you will remember to use for those spells. If you don't care, just pick and get use to it or use stooves suggestions.
Here is the video as promised:
Please, if you have any other specific questions, just ask I'll be happy to help.
