Insane in the Membrane FOUR SEPARATE TIMES across both the NA and EU regions and both versions of the game (Retail and Classic).
Four Times the Grind, One Title: “The Insane”
Reddit user LaPetiteBoutique915, who goes by the in-game name Dèl, has shared a screenshot with all his four characters proudly wearing The Insane title. One on EU retail, one on US retail, and one each on EU and US Classic realms.
Why is This Achievement Truly for the Insane?
For Insane in the Membrane, you must raise your rep to Honored with the Bloodsail Buccaneers, and Exalted with Booty Bay, Everlook, Gadgetzan, Ratchet, Darkmoon Faire, and Ravenholdt.
Farming Bloodsail Buccaneers rep is a slow painful grind that decreases your reputation standing with the Steamweedle Cartel.
On top of that, you will need around 1,400 
Heavy Junkbox acquired via Rogue Pickpocketing to earn Exalted with the Ravendholt.
Dèl took the punishing journey not once, but FOUR times. This is not just dedication, it’s a cry for help or a standing ovation… or BOTH!
