Build Introduction
The rabid werewolf is relentless, rapidly infecting enemies before rupturing them in an explosion of poisonous ruin.
The Lacerate Rabies Werewolf has a chain-reaction gameplay loop: Apply
Rabies Superinfection to spread Poisoning, then critically detonate the accumulated Poisoning damage with
Lacerate Toxin Outburst. While Werewolf gameplay is often about choosing between Direct Critical Strikes or Damage over Time, this combo allows us to leverage both ways at once.
Just starting on your druid adventure? Check our Druid Leveling Guide for options on how to level, and swap to this one at Level 70, after grabbing the Required Unique Items for the build to function.
Strengths and Weaknesses
- werewolf build for werewolves
- fast and mobile gameplay
- high damage combo attacks
- melee range combo setup
- werewolf builds need significant defensive investment
- several unique items required for function
Build Requirements
The following Unique Items are required for build functionality to get started on the Rabies Lacerate Druid:
Wildheart Hunger is the
Rabies Unique Item, allowing your rabies’ Poisoning to spread faster and ramp up damage as it spreads. The build can function without this Unique, but will be slightly worse for it in most situations.
Mad Wolf’s Glee removes the cooldown of Lacerate and makes Lacerate a Core Skill, allowing successive casts without any worry about cooldown reduction.
Dark Howl makes
Debilitating Roar a Werewolf Skill, which preserves your Werewolf Form. Dropping your form removes your Rush of the Red Wolf Moon Talisman Set buffs, so it is required for function.
This is a Spirit-intensive build; it is recommended to have a
Ring of Starless Skies when beginning this build.

Blood Howl is triggered with
Malefic Crescent and is not required on the Skill Bar.
Earthen Bulwark is triggered with
Que rune, and is not required on the Skill Bar.
In earlier gearing, use
Spirit Howl on
Blood Howl for sustaining Spirit Generation.
Eternal players complete Renown events in each zone for additional Skill Points, while Seasonal Players engage with the Season Journey and Rank for these Skill points.
Druid Class Mechanic – Spirit Boons
Use these Spirit Boons for the Rabies Lacerate Druid.
| Boon Name | Animal | Effect |
|---|---|---|
| Gift of the Stag | Deer | Gain 30 Maximum Spirit. Gain 5 Spirit Per Second. |
| Iron Feather | Eagle | Gain 30% Maximum Life |
| Avian Wrath | Eagle | Gain 20% Critical Strike Damage. |
| Energize | Wolf | Lucky Hit: Dealing damage has up to a 15% chance to restore 15 Spirit. |
| Masochistic | Snake | Lucky Hit: Critical Strikes with Shapeshifting Skills have up to a 30% chance to grant Berkserking for 4 seconds. Berkserking increases the damage of Shapeshifting Skills by 10%x. |
If your Spirit Generation is sufficient, you can swap Energize for Bolster.
Calamity Spirit Boon is not taken because it would make Lacerate too long to upkeep your
Ring of Starless Skies buff.
Druid’s primary resource is Spirit. See the Spirit Management Section on how to keep casting!

Legendary Aspects
Gear Slots with multiple options are listed in recommended priority. Any slot with a Mythic Unique will include a valid alternative to use while still leveling and gearing. Combine these with the right stat priorities.
| Slot | Aspect | Unique/Legendary Aspect Power |
|---|---|---|
| 2-Handed Weapon | Writhing Aspect | Enemies take 7.5-15% (15-25%) increased Damage Over Time for every second they have been affected by a damage over time, up to 10 seconds. |
| Helm | Mythic – Aspect of Glynn’s Anvil | Gain 20% Damage Reduction and +4 Ranks to All Skills. |
| Chest | Unique – | |
| Gloves | ||
| Pants | Aspect of Debilitating Toxins | You gain 30% Damage Reduction, and Poisoned Enemies deal 15% less damage to you. |
| Boots | Unique – | Rabies spreads 100% faster, and deals 10% bonus damage (max 120-150%) each time it spreads. |
| Amulet | Kulle Transfiguration – Overcharged Aspect | |
| Ring 1 | Crushing Aspect | While Fortified, you deal 45-65% increased damage |
| Ring 2 | Mythic – Balanced Aspect | Spending your Primary Resource reduces the Resource cost of your Skills and increases your damage by 10% for 3 seconds, up to 50%. |
| Unique Charm | Gain 15-20% bonus Poisoning Damage. Direct Damage from Werewolf Skills gain a bonus equal to 50% of your Poisoning Damage, up to 50%. Poison Damage from Werewolf Skills gain a bonus equal to 100%x of your Bonus Attack Speed and Critical Strike Chance, up to 50% |
Before acquiring
Mark of the Old Wolf on Unique Charm or a seal with 6 slots, it can be used as a ring.
Before acquiring
Overcharged Aspect on Kulle Transfiguration on the Amulet, it can be placed on pants or legendary boots.
For Pants, between defensive options like
Aspect of Heavenly Strength, Debilitating Toxins or
Aspect of Disobedience, you can choose whichever you have at the highest salvage rank. Each is usable. Heavenly Strength has the best upfront Damage Reduction. Disobedience can be the highest total Damage Reduction, but requires a ramp-up of stacks.
Note on
Aspect of Glynn’s Anvil: This can be a powerful source of Damage Reduction. However, to get the most of it, you would have to trade off Defensive Temper Max Life for Max Resolve Stacks. At very good gear, this may be better than other Defensive options.

Stat Priority and Tempering Affixes
Each line of affixes is listed in order of importance, and some alternatives are given. The bolded affixes are the most important targets for Masterworking upgrades. Unique Items cannot be Tempered, but can be Masterworked. Check the Tempering guide and Masterworking guide for more details about these topics.

Remember to fully Socket, Temper, Enchant, Aspect, and Masterwork (STEAM) your item before Transfiguring it in the Horadric Cube!
Critical Strike Damage Multiplier gear affix is exclusive to Damage over Time Multiplier. It is best to get a mix of both for the highest damage potential, slightly favoring Damage over Time.
Shapeshifting and Werewolf are the best tags for Skill Ranks, as that also affects
Debilitating Roar and
Blood Howl.

After gearing, look at progression systems like Paragon next.
Uniques and Mythic Uniques
The following Unique Items are recommended for the Lacerate Rabies Druid.
Mad Wolf’s Glee makes
Lacerate a Core Skill, removes the cooldown, makes the Skill cost 100 Spirit, and removes the Immunity on cast. This allows us to freely cast Lacerate in succession after applying our Rabies.
Wildheart Hunger is not strictly required for the build to function, but spreads Rabies faster and increases the damage of Rabies each time it spreads. Utterly fantastic in multitarget to ramp up toxic outburst targets, skippable in single target bossing. For those situations, keep Legendary Boots with a useful utility on hand.
Dark Howl gloves give a strong enough multiplier to justify use. As well, it codes
Debilitating Roar to be a Werewolf Skill, which allows use while still maintaining Werewolf form, which is essential for keeping our Talisman set Rush of the Red Wolf Moon bonuses active.
Malefic Crescent is an interesting option. The multiplier is worse and it may be harder to get good affixes on a Unique item. However, permanent Blood Howl for the dodge, extra Unstoppable, better Berserking, and some small Poisoning Over Time could be a valid play. It is something to test.
Mark of the Old Wolf is our Unique Charm choice. The Poisoning boosts Rabies damage, the Direct Damage boosts the Lacerate
Toxin Outburst, and the Poison boosts Rabies again! This is one of the few builds that gets full benefit from all three parts of the Unique Item.
Mythic Unique Items
The following Mythic Unique Items are situational to the build:
Harlequin Crest – Extra ranks in all skills will be excellent for boosting Rabies damage, and also to give boosts to our Defensive Skills. The Damage Reduction on top will help us stay alive in our squishy squishy squish squish werewolf life. Awoo? more like AH! OW!
Heir of Perdition is always on the table for those looking to get the highest damage option. However, make sure you are confident in your survival with strong gear. Rabies Lacerate already does large damage, quickly outpacing its toughness.
Ring of Starless Skies is a powerful Resource Cost Reduction tool and damage multiplier with excellent Gear Affixes. It is also a significant source of our total Attack Speed and Critical Strike Chance. The Core Stats boost up both
Rabies and
Lacerate. A well-statted Legendary ring with multiplier affixes like Critical Strike Damage or Damage over Time Multplier has the potential to do better top-end damage than Starless. Still, Spirit will need to be very well managed to use one.
Gems and Runewords
The following are the recommended Gem and Runeword setups for the Rabies Lacerate Druid:
Gems
| Weapon Gems | Armor Gems | Jewelry Gems |
|---|---|---|
| 2x 1x |
Most of the dangerous one-shots are fire or lightning damage. Swap in more Rubies or Topazes if necessary.
Runewords
Use these Runeword combinations for the Rabies Lacerate Druid, placed in Armor slots:
| Runeword Combination | Runeword Effects |
|---|---|
| Neo Vex | |
| Igni Que | Evoke the Druid’s |
Runewords use a conditional Rune of Ritual to generate Offering. The Rune of Invocation consumes the Offerings to trigger its effect. Runes replace Gems in gear with two sockets. Up to two sets of Runeword Pairs can be placed at a time.
Using
Que allows us to benefit from our Skill Point investments into
Earthen Bulwark without needing it on the Skill Bar.
Vex will increase Skil Ranks of all Skills, and stacks up to +3. This includes boosts to Rabies damage and a welcome boost to our Defensive Skills.
Paragon Board
The following are the preferred Glyphs, Boards, and pathing recommended for making the most of the Rabies Lacerate Druid.
The road to max Paragon is long. The above Paragon tabs show the following:
- The first tab shows a setup around Paragon 200. This pathing is stricter to grab all Legendary nodes, essential rare nodes,s and to fill in glyph areas.
- The second tab is the paragon tree in its final form.
At level 15, the glyph grows in radius and gains a special effect. At level 46, the glyph grows to its max radius and gains a strong conditional damage multiplier.
Total Boards placed are now limited to a max of 5; the Starter Board and four Class Boards. Rotate boards as necessary.
Werewolf is taken over
Headhunter for extra defense. Those can be exchanged once your gear is better.
If you need even more defense,
Fang and Claw can be swapped for
Undaunted. Take nearby Int nodes.
Season Mechanics
There are no specific seasonal mechanics in season 13. Instead, the Lord of HatreDLClc and all of its features are available to explore. This includes:
- War Plans
- New Skill Trees
- Talisman Sets
- The Horadric Cube
- Echoing Hatred
Enjoy the expansion!
Talisman
Lord of Hatred introduces Talisman sets. The Talisman is a Seal and the Charms placed within. Charms have general affixes from a limited pool and can have Set bonuses, which grant significant power.
The ideal setup for an Ancestral Seal for the Lacerate Rabies Druid is the following:
- Seal Bonus +1 Charm Slot for 6 total Charm slots. This allows for a full 5-set and Unique Charm.
- 5/5 Rush of the Red Wolf Moon Set
- Unique Charm for
Mark of the Old Wolf
Ideal affixes for the Seal, Seal Bonuses, and Charms include the following in order of priority:
- Ranks to Shapeshifting, Core, Werewolf, or Wrath Skills for more
Rabies damage. Shapeshifting and Werewolf will also boost
Debilitating Roar and
Blood Howl. - All Stats. All Stats is important for Paragon Secondary Bonus nodes.
- Armor or Max Life.
- Critical Strike Chance
The Rush of the Red Wolf Moon is the werewolf form set for the Druid. The 2 set bonus ramps up a stacking Frenzy by targeting the tethered enemy with damage. After enough stacks, the 5 set bonus replaces the 2 set multiplier with a general multiplier and gives your Werewolf skills a Dash.
With the Rabies Lacerate Druid, this means Rabies and Lacerate will receive a Dash that requires a target. In a silly turn of events, because of
Dark Howl, Debilitating Roar also gets a Dash, which teleports you to your cursor location on cast.
Mercenaries
Mercenaries are followers who aid the Player. A main mercenary is chosen to fight alongside you, granting strong buffs or perks, and a secondary Mercenary is chosen to periodically Reinforce you.
The following Mercenary options are recommended for the Lacerate Rabies Druid:
Main Mercenary – Raheir, the Shieldbearer
- Skills:
Shield Charge,
Raheir’s Guard,
Provoke,
Mocking Lure
Reinforcement – Aldkin, the Cursed Child
- Reinforcement Ability –
Field of Languish - Opportunity –
Rabies
Raheir will provide a small defensive boost and help tank for you. Other options are also valid, such as using Subo for minimap tracking quality of life.
Build Mechanics
Rotation and Playstyle
- Activate your
Hurricane. - Upkeep your
Debilitating Roar for Life and Damage bonuses. - Apply
Rabies to enemies in a cluster. Multiple applications of Rabies from additional Casts can stack! - Once enemies are Poisoned with
Rabies,
Lacerate to explode the Poisoning damage in the area.
Blood Howl will be automatically cast with
Malefic Crescent.
Earthen Bulwark will automatically be cast with
Que.
Targeting your Blood-Tethered enemies from the Rush of the Red Wolf Moon Talisman 2 Set bonus will ramp you into your Feral Frenzy 5 set bonus sooner, allowing your Werewolf Skills to dash, greatly increasing damage and mobility.
Skills, Support, and Synergies
The Lacerate Rabies Druid is a combo execution build:
Rabies with Superinfection variation applies a spammable, heavy Damage over Time that slowly spreads to other enemies.
Lacerate, with the Toxin Outburst variation, explodes any present damage over time. This explosion can Critically Strike, which is a large quad scaler of the base Poisoning damage.
Rabies is a Core Skill with the
Superinfection variation, allowing it to be cast without cooldown for a spirit Cost so that it can be quickly spread to enemies. However, it is the closest range Skill we have, so expect to be close and in the thick of things.
Rabies spreads to one target at a time, and a cast cannot go back to reinfect the same target. However, multiple casts of Rabies can stack and overlap with one another for a large base amount of poisoning ticking away on enemies.
Lacerate base damage does not improve the Toxin Outburst explosion, so we do not invest extra Skill points into Lacerate, which is good because that means more Points for our Defensives, which are necessary to live.
Supporting Skills
Cyclone Armor is largely just used for the passive boost to All Resistance, but the active knockback can be handy. You can also try out the Reversal variation to cluster enemies better for Lacerate burst.
Hurricane with the Derecho variation becomes a toggled skill, always active. It places Weaken on targets and helps with total damage events to trigger essential Lucky Hits like Spirit Boons or
Constricting Tendrils. This can be pricy upkeep before getting paragon like
Lust for Carnage or
Ring of Starless Skies; if you cannot maintain it, use another Skill in the meantime, or another variation of Hurricane. This means less uptime but removes the Cost. Other Options can include
Ravens for Vulnerable, Resolve, or
Nagu count.
Blood Howl grants Dodge Chance, Movement Speed, or Unstoppable, and has a couple of variation options. Early on,
Spirit Howl can help keep the spirit-hungry build going. Later,
Berserking Howl gives a minor damage buff, Berserking upkeep, and a damage-over-time applier for a bit more damage on our Lacerate outbursts and to help apply Poisoning even sooner for Debilitating Toxins Aspect. We trigger this off the bar with
Malefic Crescent.
Debilitating Roar grants Damage Reduction and Maximum Life. With
Dark Howl, it becomes a Werewolf Skill, so it does not break our Form, which is important for keeping Talisman Set buffs active, and of course, thematically. Long live just staying in one Form. Extra ranks from Charms or other gear that put the Skill beyond 15 can increase the duration past the cooldown, meaning those excess points can be placed elsewhere.
Wolves give a large amount of Attack Speed on Critical Strike, and help tank a bit for you. Lacerate does not speed up its strikes from Attack Speed, but you will be able to layer your
Rabies faster.
Hurricane, Cyclone Armor, and Wolves are all Versatile Skills, meaning they just keep the previous Form when cast.
With
Que, we gain access to
Earthen Bulwark without placing it on the Skill Bar. Earthen Bulwark provides Fortify, periodic Unstoppable, Barrier, and Resolve. Excess ranks beyond 100% Absorb are wasted, as the Barrier cap is 100% of your health. Any Skill points from extra skill ranks past that can be reallocated elsewhere.
Buff Check
Here’s a breakdown of where we are getting what essential buffs!
| Power | Source |
|---|---|
| Barrier | |
| Berserking | Spirit Boon Masochistic |
| Crowd Control | Talisman Charms, if any |
| Ferocity | |
| Fortify | |
| Poisoning | |
| Resolve | |
| Unstoppable | |
| Vulnerable | Overcharged Aspect |
| Weaken |
Spirit Management
Rabies and Lacerate are Spirit Hungry Skills with a high base cost. There are no longer tempers for Resource on Ultimate or Wrath Cast to handle that anymore, so we can squeeze in some other sources.
- Gift of the Stag Spirit Boon gives 5 Spirit Generation.
- Energize Spirit Boon gives a lucky hit chance to restore spirit.
Lust for Carnage gives Critical Strike Chance on cast.
Ring of Starless Skies gives significant Cost Reduction. - Ring Tempers for Resource Generation.
- The
Ancestral Guidance board offers strong resource generation, max spiriand t, spirit on kill. - Any Spirit Generation on Armor slots.
If this is not sufficient for uptime or while still gearing, we can optionally use one or more of the following:
- Gear extra Resource Cost Reduction, Resource Generation, or Spirit Generation on gear, as available.
- Use the
Spirit Howl Variation for
Blood Howl. - Use
Balanced Aspect on the ring slot for a silly amount of Spirit Generation per second after casting a skill with a cooldown. Our Cooldown skills are Debilitating Roar and Cyclone Armor.
Farm Content
In farm content, where applying single spread
Superinfection Rabies can be annoying, we have the option to use
Blood Howl
Berserking Howl Poison as our Poisoning for Lacerate. Take points from
Cyclone Armor and place them to max out 15/15 Blood Howl. This should scale well enough to farm with.
The Relapse Variation of
Rabies is also an option, spreading to more targets at a time for more damage.
Season 13 Updates
Rabies on my Skill Bar? Finally.
Please enjoy the Rabies Lacerate poison detonation combo interaction build. For a full breakdown of the gameplay, including extra gameplay options, be sure to check the Build Mechanics section.
You will notice many decisions for skills, unique items, runes, glyphs, and gearing are defensive-oriented. This gameplay has the damage; it just needs to live. As you gear up and level Paragon, feel free to try to swap to the more offensive options listed.
As ever, thanks for being here, you’re great!
Check the Patch Notes for a full list of updates. It is a lot. For general Season 13 information, check out our Season 13 Hub.
