Midnight’s most recent update, patch 12.0.5, introduced the Voidforge system, and one of its most exciting features is the bonus roll mechanic. Using
Nebulous Voidcore, you can spend currency after completing dungeons or raids for another shot at loot. The selling point here is that items you already received get removed from your loot pool, so over time you are supposed to get closer and closer to that specific piece you really need.
Except… that it is currently not working as it should.
Duplicate Bug
Players are already reporting that the duplicate protection is completely broken. There are plenty of posts online, and even video proof of players showing how they are receiving the exact same two-handed polearm twice in a row from back-to-back Skyreach +12 runs. Same dungeon, same key level, same item, both times from a bonus roll.
Unfortunately, that seems not to be a rare occurrence, as other players are also reporting similar experiences, including receiving the same item they got from the original loot drop, just at a higher track. The general advice from the community right now is to hold off on spending your
Nebulous Voidcores until Blizzard addresses the issue.

How the Voidforge Bonus Roll Works
According to the official patch notes, receiving an item via a bonus roll should remove it from the loot pool on a per-difficulty basis. So if you roll a trinket from a Mythic+ dungeon, that trinket should not be eligible for future rolls until every other item in the pool has been obtained. That is clearly not happening right now.
There is no official response from Blizzard yet. However, given how big a focus the Voidforge system is for gearing in patch 12.0.5, a fix should hopefully not be far off. In the meantime, save your Voidcores.
