Last night, Blizzard released a blue post talking about adjustments to the drop rates of 
Ascendant Voidshard. As of the time of posting, this hotfix is now live, and the following sources now have guaranteed Ascendant Voidshard drops!
- Heroic and Mythic raid boss kills.
- Mythic+ end-of-dungeon chests at +10 difficulty or above.
- Bountiful Delve chests at Tier 11.
- Nightmare Prey gear chest rewards.
All other content that was recommended to farm beforehand (such as Tier 1 Ritual Sites) can still drop these, but at a reduced drop rate.
