How to get the highest Item Power level gear in Diablo 4!

When reaching endgame in Diablo 4, players aim to acquire the best possible gear available in the game. There are a few secrets to obtaining top-tier gear. Currently, the highest Item Power level you can find on an item is 925. However, there are rare unique named mobs scattered throughout Sanctuary, found in specific locations, that have a chance to drop items with a higher Item Power level.

These Rare items can be upgraded like any other Rare (yellow) item, but at a much lower cost compared to others. They usually come with specific stats, and are highly valuable for players seeking the highest Item Power level, as they can be transformed into Legendaries.

In this guide, we will cover each zone and the corresponding enemies that can be found there. We will also provide detailed information about the items they drop.

It is worth noting that these unique rare Elites have higher health and deal more damage than normal Elites. They also provide more XP. Defeating them not only poses a challenge but also contributes to in-game challenges such as the Poacher and Trophy Hunter challenges.

The Item Power level range of these Rare items varies based on your character’s level when you defeat the Elite, as well as the world tier you play in. You can obtain the item each time you defeat the enemy, and can replace it with a higher Item Power level version as you level up.

Additionally, it is important to remember that the Item Power level will vary with each drop. Therefore, it may be necessary to defeat the enemy multiple times to obtain the desired Item Power level.

Remember to verify the location on the map by referring to the marked area. This step is important because, in some screenshots, the mouse cursor may be hovering over a different spot on the map, so the area name (at the top) may not be relevant. Instead, focus on the highlighted area.

Fractured Peaks

There are 4 very rare enemies in total that can be found throughout the Fractured Peaks zone.

Two-Handed Staff (for Sorcerer)

Staff of Elemental Command
– Damage to Crowd Controlled Enemies
– Lightning Damage
– Fire Damage
– Cold Damage

Characteristics: Fire and duplication Sorcerer. The NPC will summon two clones that also shoot fire.

Mace (for Barbarian)

Mace of Blazing Furor
– Overpower Damage
– Damage to Burning Enemies
– Damage while Bererking
– Overpower Damage

Characteristics: Huge, tanky NPC with a Mace that shoots fire at players.

Ring of Splintered Wood

Characteristics: Huge Tree that inflicts poison damage and spawns poison spiders, leaving acid on the ground.

Scosglen

There are 4 very rare enemies in total that can be found throughout the Scosglen zone.

Amulet

Blind Man’s Bell
– Resistance to All Elements
– Control Impaired Duration Reduction
– Damage Reduction while Injured
– Movement Speed for x seconds after killing an Elite
– Damage after killing an Elite

Characteristics: Blind Vampire that will only spawn after the player killed surrounding enemies.

Manhunter’s Breeches
– While Injured, your potion also grants Movement Speed for x seconds
– Ranks of Rabies
– Damage after killing an Elite
– Healing Received

Dropped by: Garbhan Ennai
Characteristics: The Butcher of Cerrigar. He will only spawn at night, inflicting Vampiric, and multiple other status effects, as well as transforming into a Werewolf once he lost half his health (bar).

Characteristics: Probably the only friendly enemy players will encounter. He is a Monk who refuses to fight, but he can be attacked and killed.

Characteristics: The Duke of Shadow. Looks like a big Balrog. Only appears during the night. Has two forms: one Demonic, and one Human called Jebtaa, which appears once the Demonic form is defeated.
