All Faceless Masks in Horrific Visions (Patch 11.1.5) – Effects & Unlock Methods
Faceless Masks are optional difficulty modifiers that players can use to increase the difficulty of Horrific Visions as well as the rewards. The following guide covers how to unlock all the masks and what effects they bestow upon your character.
Introduction
Faceless Masks are optional challenge modifiers you can activate in Horrific Visions inside the safe room to significantly ramp up the difficulty and your rewards. Patch 11.1.5 has expanded the system to include 8 total masks, giving players more ways to tailor their runs! Before you put on a mask, you must first unlock it. Every mask comes with a different negative effect that makes your run harder.
What do Faceless Masks Do?
Each mask you equip will:
- Apply a unique gameplay challenge (negative effect that makes your run more difficult).
- Increase enemy health and damage by 25%.
- Increase the amount of
Displaced Corrupted Mementos earned by 20%.
All effects stack for each mask equipped. Completing a run with 8 Faceless Masks active will increase enemy health and damage by 200%
and you will receive 160% more Displaced Corrupted Mementos. Keep in mind loot caps out at Hero 4/8 at 5 Faceless Masks.
Faceless Masks and How to Unlock Them in Patch 11.1.5
Below, you will find a list of all Faceless Masks available in Patch 11.1.5, the unique gameplay challenge they activate when you put them on, and how to collect them.

Reduces your maximum Sanity by 50%. | Complete all objectives in a single Horrific Vision run (no mask required). |

Enemies deal 400% more Sanity damage to you. | Clear the Valley of Honor in Orgrimmar with at least 1 mask active. |

Two Madness effects are active in each area instead of one. | Clear the Old Town district in Stormwind with at least 1 mask active. |

Moving causes void zones to spawn in your path for 10 seconds. Standing in them drains 30 Sanity per second. | Clear the Valley of Wisdom in Orgrimmar with at least 1 mask active. |

While below 50% Sanity, a Void enemy will occasionally spawn and attack you. | Clear the Mage Quarter in Stormwind with at least 1 mask active. |

Killing enemies will Bolster their nearby allies with Vengeance, increasing their damage done by 15% for 20 seconds. This effect stacks. | With 1 mask active, loot the mask from Vengeful Voidspeakers fond between Old Town and the Trade District. |

Summons Hogger or Gamon every 25 enemy kills. Critters do not count. This nemesis has Sanity Breaker abilities, and awards Sanity via the Elite Extermination Tribute upgrade when killed. |

How to Equip and Unequip Faceless Masks
When you are inside the Horrific Vision safe room, interact with the individual altars to put the masks on/off one by one. You will see your current debuffs indicated in your UI.
